Once the “Needs Analysis” is completed and the desires of the client(s) are known then a “Continuum of Care Plan” can be developed to give us a road map for the future.

The “Continuum of Care Plan” is a document that gives options for the client's long-term care. The client and his/her family can decide to implement one part, all parts, or no parts of the plan.

For example, if the loved one wishes to (and is able to) stay at home then we would look at in-home assistance, community resources, and home maintenance.

Of course, the plan is only as good as the information that has been shared. For example, some eligibility programs have financial thresholds and if all income data was not provided then the recommendation may be in error.

My goal with the Care Plan is to help each client continue to have the highest quality of life possible and to respect their dignity.
